This application form helps us check eligibility for the Adult Swimming Programme, which is designed to support adults who currently face barriers to being physically active. Please complete all sections as accurately as possible. To be eligible, you must be aged 18 or over, face at least one barrier to physical activity, and agree to the use of anonymised feedback for monitoring and reporting purposes in line with Sport England guidance. All information you provide will be handled securely and confidentially.
